'Image (Capable)' macro sits on a page showing 'No Images Selected'

What you are seeing

An 'Image (Capable)' macro sits on a page showing 'No Images Selected'

#Why it happens

Somebody inserted the placeholder macro and cancelled the picker. The card stays on the page and is visible to readers, not just the author, and the picker will not reopen automatically for the rest of that browser session.

#What to do

Either click 'Select and insert images' to finish the insertion, or delete the macro from the page. Note that unlicensed and anonymous readers see nothing there at all, because this macro has no unlicensed access grant.
