Forms are only available in ‘Capable for Confluence Advanced’
#1. Add a form macro to a page, assign a name and ID
Add a form macro to enclose your fields in, so that they can be validated before being saved. This ensures that all the input data is properly checked for correctness and completeness, preventing any invalid or incomplete information from being submitted and stored.

#2. Add input macros inside the form
Input macros are the fields that let you ask for information from users in a structured and interactive way.
These fields are designed to collect specific data by prompting users to provide the required details, making it easier to gather accurate and relevant information.

#3. Create templates with forms
Templates make it easy to create new form pages based on other events happening (with automation), from ‘Create from template' button macros or from the ‘new page’ screen.

#4. Create new pages from the template
Make your forms extensible and easy to create by using Confluence templates, which provide a flexible and user-friendly way to standardise and streamline form creation across your team or organisation.

#5. View Form Responses and Export
You can view all form responses in a central view and export the results to CSV for processing.

You can even use Confluence Automation to create pages from templates with the form on them.
