Versions are missing from Version history
What you are seeing
Some versions are missing from Version history.
#Why it happens
Version history hides any version saved within 15 seconds of the one shown before it, so rapid auto-saves don't flood the list. It also shows only the last 50 entries, and tells you "Limited to the last 50 entries".
#What to do
Nothing is wrong. Both behaviours are deliberate.

#A few things that catch people out
Version history stages a version rather than restoring it. It also hides versions saved within 15 seconds of each other and never shows more than 50.
