Form ID | empty | The stable identifier for this form. Drives the Forms catalogue, the capable_forms_form_ids CQL field, the capable_forms_{formId} page label and the CSV filename. |
Form Name | empty | Human-readable title shown as the form's heading on the page, in the Forms catalogue, in the page byline and in the search panel. Falls back to the Form ID, then to "Form". |
