Someone's answers were replaced by someone else's.
#Why it happens
By design. A page holds exactly one record per form, not a log of submissions. A second person filling the same form on the same page overwrites the first person's answers, and "Responded by" is replaced with the newest saver.
#What to do
Give each respondent their own page. Be aware that copying a page does not re-index the copy - somebody has to open and edit the new page before it enters the catalogue.
