Nest ‘Input’ macros inside the form macro to create forms and collect responses.

## Settings

| **Setting** | **Default** | **What it changes**                                                                                                                                                          |
| ----------- | ----------- | ---------------------------------------------------------------------------------------------------------------------------------------------------------------------------- |
| Form ID     | empty       | The stable identifier for this form. Drives the Forms catalogue, the capable\_forms\_form\_ids CQL field, the capable\_forms\_{formId} page label and the CSV filename.      |
| Form Name   | empty       | Human-readable title shown as the form's heading on the page, in the Forms catalogue, in the page byline and in the search panel. Falls back to the Form ID, then to "Form". |
