What you are seeing
A document disappeared after saving an edit, with no error shown.
#Why it happens
The save deletes the old attachment first and then uploads the new one. If the upload fails, the failure is swallowed and nothing is displayed.
#What to do
Copy the document text before saving a large edit. If it vanishes, re-create it from your copy. Check attachment size and Jira attachment permissions before retrying.

If the delete succeeds but the re-upload fails, the document is lost and no error is shown. The failure is swallowed.
