# Save appeared to succeed but the diagram is now missing from the issue entirely

**What you are seeing**

A save appeared to succeed but the diagram is now missing from the issue entirely.

## Why it happens

On edit, the old attachment is deleted before the new one is uploaded, and an upload failure is not surfaced. A failed save can lose the original.

## What to do

Reload the issue to confirm the current state before assuming a save worked, and keep a copy of large or important diagrams. Retry the save; if it keeps failing, check that you still have permission to add attachments to that issue.
